1. Acceptance of Terms
By creating an account or using the Smplii Pilot application ("the app"), you agree to these Terms and Conditions. If you do not agree, do not use the app.
2. Description of the Service
Smplii Pilot is a limited pilot program providing a post-discharge care assistance tool for patients and family caregivers. The app helps track medications, vital signs, appointments, and recovery activities following hospital discharge.
3. Not Medical Advice
The app does not provide medical advice, diagnosis, or treatment. Content generated by the app — including AI-generated summaries or suggestions — is for informational purposes only and may be incomplete or incorrect. Always consult your healthcare provider for clinical decisions. In an emergency, call 911.
4. SMS Messaging Program
Program name: Smplii Pilot Authentication
Description: We send one-time passcodes (OTPs) to verify your identity when you sign in to the app. No marketing or promotional messages are sent.
Message frequency: Varies based on sign-in activity.
Message and data rates may apply.
To opt out, reply STOP to any SMS from us. You will receive a confirmation and no further messages will be sent. Note that opting out will prevent receipt of verification codes and may limit your ability to sign in.

Supported carriers are not liable for delayed or undelivered messages.
5. SMS Caregiver Notifications Program
Program name: Smplii Caregiver Notifications
Description: We send care coordination notifications to caregivers of post-discharge patients on the Smplii platform. Messages include caregiver invitation confirmations and care updates related to the patient you support. No marketing or promotional messages are sent.
Message frequency: Varies based on care activity. Typically 1–5 messages per month.
Message and data rates may apply.
To opt out, reply STOP to any SMS from us. You will receive a confirmation and no further messages will be sent.

6. Eligibility
You must be 18 years of age or older to use the app. By using the app, you represent that you meet this requirement.
7. Your Account
You are responsible for maintaining the security of your account. Notify us immediately at privacy@smplii.com if you believe your account has been compromised. We are not liable for losses resulting from unauthorized access to your account.
8. Caregiver Access
If you invite a caregiver, they will have access to the same information visible to you in the app, including medications, vitals, notes, and appointments. You may remove caregiver access at any time from Settings.
9. Data and Privacy
Use of the app is also governed by our Privacy Policy, which is incorporated into these Terms by reference.
10. Pilot Participation
This is a limited pilot. We may modify, suspend, or discontinue the app at any time. We will make reasonable efforts to notify participants of material changes.
11. Limitation of Liability
To the fullest extent permitted by law, Smplii is not liable for any indirect, incidental, or consequential damages arising from your use of the app. Our total liability for any claim arising from use of the app is limited to the amount you paid to use it (if any).
12. Changes to These Terms
We may update these Terms as the product evolves. The "Last updated" date at the top of this page will reflect any changes. Continued use of the app after changes constitutes acceptance.

14. Governing Law
These Terms are governed by the laws of the State of Delaware, without regard to its conflict of law provisions. Any disputes arising from use of the app will be resolved in the courts of the State of Delaware.
